(UPDATED) Since February 3, 2021, the City Government of Davao City mandatorily required all establishments within its jurisdiction to use the Safe Davao QR Code system per Executive Order No. 2 Series of 2021.
All establishments are advised to ensure they download the mobile scanner. Meanwhile, individuals, either residents or non-residents, must have their own Safe Davao QR Code and present it upon entering establishments and traveling around the city.

What is Safe Davao QR Code?
The Safe Davao QR Code is the platform created by the City Government of Davao to expedite contact tracing efforts and enforce the prohibition of non-essential travel in Davao City. It is Davao City's official contact tracing system that aims to protect Davaoeños from the risks of COVID-19 infections.
Said system was implemented way back on November 23, 2020, as a scanning point for selected establishments within the city. Currently, it is required for ALL individuals and establishments in Davao City to use the said contact tracing system. It also acts as an entry pass for travelers entering the city.

What are the requirements to get a Safe Davao QR Code?

3. Fill out the Safe Davao QR Code registration form

You must fill out the Safe Davao QR Code registration form for individuals to the process of getting your own and unique personal quick response (QR) code. To do this, you must fill out the following information:
- Your complete name;
- Date of Birth;
- Primary mobile number;
- Secondary mobile number (optional);
- Landline (optional);
- Email address; and
- Complete address.
In the address section, you must select from the following: resident or non-resident. Select resident if you're from Davao City and non-resident if you're not from Davao City.
Once you are done filling out the registration form for your Davao QR Code, you may now click on the "NEXT" button to proceed.
Note: Before you fill out the registration form, be sure to prepare your active mobile number to get your one-time-pin (OTP) which is needed to complete your QR Code registration. Also, you must have a digitized copy of the requirements which you need to upload.
4. Upload necessary documents

As mentioned above, once you filled out the Safe Davao QR Code registration form, you must upload the required documents to continue. Just click on upload photo for each requirement: (1) photo of your face, (2) photo of your valid id, and (3) photo of you holding a valid ID beside your face.
After you upload the documents, you must put a checkmark in the box authorizing LGU Davao to collect your data for contact tracing purposes. Also, answer the captcha or place a checkmark on the box beside the "I AM NOT ROBOT" text.

Now that you are done with the required details and information, a pop-up message will appear requiring you to review the details you provided. Click on the "ALL CORRECT" button to continue your registration. Afterward, you must click on "SEND OTP NOW" to receive your one-time password (OTP), which is needed to get your Davao QR Code.

5. Enter OTP and receive your QR Code

Finally, once you receive the 6-digit OTP code, you must input it in the required field to verify your registration. By this point, you will receive your Davao QR Code. You can now use it to travel within Davao City and enter establishments of your choice.
